*Instructions for "Acknowledgement of Policies and Procedures": After reviewing the policies listed below, scroll down to the BOTTOM of the page and complete all the required fields. Then, click the button that reads "Submit Signature Acknowledgment".
MEDICATIONS: All medications are kept in a locked cabinet and are not accessible to the student(s). Medications will not be accepted without parent consent and the signed "Authorization for Medication Form." Parents are responsible for informing us if child is allergic to any kind of over-the-counter medication, otherwise, parents will authorize over-the-counter medication.
For medications prescribed by a physician(not over the counter) with specific instructions, parents must complete an individual "Authorization for Medication Form" for each medication. This indicates name of medicine, time administered and dosage given. All dosages given are monitored by the staff or faculty member administering it. A witness will be present each time a dosage is administered.
Students are not permitted to administer their own medications nor to carry medications in their book bags or personal belongings. All medications must be turned into the front office for safe keeping and administration of dosage.
