​INFORMATION, RULES & POLICIES

All students must have on file the following:
  • Acknowledgment of having read "Information Manual for Parents & Students";
  • "Student Required Forms Packet" below must be signed and submitted;
  • Birth Certificate and all completed Immunization and Medical Records must be on file;
  • New Students coming from other schools must also have their complete transcripts, standardized test scores, and any Psychological Reports on file prior to the first day of school.
​NOTE: All forms must be submitted for EACH student.

Forms must be received by NO LATER than Thursday, August 10, 2023.

To complete the acknowledgment, be sure to fill out the required information on the last page and click "submit".
