REQUIRED ACKNOWLEDGEMENT OF IMPORTANT
INFORMATION, RULES & POLICIES
All students must have on file the following:
Forms must be received by NO LATER than Thursday, August 10, 2023.
- Acknowledgment of having read "Information Manual for Parents & Students";
- "Student Required Forms Packet" below must be signed and submitted;
- Birth Certificate and all completed Immunization and Medical Records must be on file;
- New Students coming from other schools must also have their complete transcripts, standardized test scores, and any Psychological Reports on file prior to the first day of school.
