REQUIRED ​DOCUMENTS
​AND PARENT ACKNOWLEDGMENTS 
 OF IMPORTANT INFORMATION, RULES & POLICIES

All students must have the following REQUIRED DOCUMENTS on file prior to 1st day of school:

• Acknowledgment for "Information Manual for Parents & Students" including school policies & procedures (below)​
• "2025 REQUIRED FORMS" (below)
• ​Acknowledgement of "NEW iPad Installation Requirements" Form (below)
• Birth Certificate (for all Preschool thru Kindergarten students; 7th Grade students)
• Updated Immunization (for all Preschool thru Kindergarten students; 7th Grade students) and Medical Records documents
• New Students: Students transferring from other schools must also have submitted their complete transcripts including all report cards, standardized test results, and any Psychological Educational Reports prior to the first day of school.

​NOTE: All forms are required for EACH student.

Forms and all student documentation must be received by NO LATER than Friday, August 1, 2025.
