All students must have on file the signed required forms below in order to be allowed to attend class. Please click on each FILLABLE link below, complete the forms, and submit them.  All forms must be received by NO LATER than Friday, August 12, 2022.

• Technology, iPad, Social Media and Cell Phone Policies Form
• Student Personal Information Form
• ​Grades K to 8 Installation of E-Books on iPad Form
• Student Video/Photo Consent Form
• Credit Card Authorization (optional)

All students are also required to have a copy of their Birth Certificate and all completed Immunization and Medical Records inside their files. New students coming from other schools must also have their complete transcripts, standardized test scores, and any Psychological Reports on file prior to the first day of school.
